If she really likes the band, the cd is a good idea. But add to it something more personal. The card is what I was also going to suggest, but you could write your message inside the cd booklet as well, so that opening it, it'll remind her of you. Be brief, but original, make her feel the message you're writing in it is especially for her, that it's not one of those phrases you hear everywhere. Make her feel special with a thoughtful little something you invented for her. You could also invite her somewhere for her birthday. Some girls like when you decide to invite them out in particular occasions. You could go drink something, have a hot chocolate, whatever she might like (you know her, so you know her tastes too). Bring her to a place she could enjoy and give your gifts to her when you're both alone. Choose the right moment. It's not nice, I think, to give gifts to somebody when there's too many people around, it's too distracting.
