As for 2, are you playing on a PS2? As if you are, it won't go past that bit of the credits so you can save after completing a game. You need to finish it on a PS1 to save.
As for 1, I'm pretty sure he dies from FOX Die, although I've heard many people think that it really is Fox in stealth, as the long Fooooxxx isn't in reference to FOX Die but rather to Grey Fox instead
