Hmmm... I'm not very sure myself these questions are answered in the anime - haven't read the manga yet. I think all of your doubts can be explained by the fact the actions and decisions of GANTZ are somewhat random, and there are no exact rules valid for every mission or every character involved. The confusion experienced by the characters in the story derives from the absence of these rules and what they seem to acknowledge after each mission isn't very helpful since the rules can be arbitrarily changed.

BTW, I've read somewhere they were planning to do another series, since the first two were produced while the manga was at a very early stage. Around 20 volumes of the manga have been released so far, so the material is now available, and the creators could use it to change the impression the first (temporary?) ending was very rushed and incomplete. The fact the anime story had to differ a lot from the manga is also because the manga was still in early production, so I don't know if they really are going to make another series, or if they'll try to make it similar to the manga from that point on or not.
